What is Letdown-Pushdown Rhinoplasty?
The letdown rhinoplasty(lowerind) technique, a form of rhinoplasty, is primarily used to correct a slightly arched nasal structure, restoring it to a smoother, more natural appearance.
In this technique, bone is removed from the side walls of the nasal bone without altering the natural structure of the nasal bridge. This allows the nasal bridge to be lowered in layers, achieving the desired shape while maintaining the integrity of the nose’s natural contours.
One of the advantages of letdown rhinoplasty is that it preserves the natural appearance of the nasal dorsum, so the way light and shadow play on the nose remains unchanged, enhancing the nose’s natural look. This technique is an evolution of the *pushdown rhinoplasty* approach, which was first introduced in the 1970s but fell out of favor for some time. Today, pushdown rhinoplasty has been revived and modified slightly, allowing for a more refined and natural result.

In traditional rhinoplasty surgeries, the arched part of the nose is corrected by removing bone and/or cartilage from the nasal bridge, often requiring the surgeon to break or file down the bone to reshape it. This method, however, can disrupt the natural anatomy of the nasal dorsum.
In *letdown rhinoplasty*, the approach is different. Instead of removing cartilage or bone from the bridge of the nose, a portion is removed from the lower side walls of the nasal structure. This allows the entire nasal bridge to be lowered, achieving a smoother profile without disturbing the natural anatomy of the nose. After excess bone or cartilage is removed, the nasal bridge, still intact, is lowered and placed within the nasal cavity.
This technique preserves the natural nasal roof, maintains the aesthetic line between the eyebrows and nose, and prevents potential irregularities that could arise from reshaping the nasal roof. By only removing the excess tissue and repositioning the nasal structure within the cavity, letdown rhinoplasty keeps the normal relationship between the nasal bones, cartilage, and surrounding tissues. It also avoids narrowing the airway, allowing the surgeon more freedom to refine the nose shape without compromising nasal function.

What is Pushdown Rhinoplasty?
Another one of the rhinoplasty applications is pushdown rhinoplasty (pushing down / pressing down) technique.Similar to letdown rhinoplasty, it aims to restore the natural contour of the nose without removing or altering the bone structure of the nasal dorsum. Instead, the entire nasal dorsum is gently pushed downward into the nasal cavity, preserving the natural anatomy. This approach maintains the natural appearance of the nasal bridge, ensuring that light and shadows on the nose remain unchanged for a more natural look.
The pushdown technique was initially developed in the 1970s. However, it has limitations due to the nasal cavity’s size, which restricts how far the nasal bridge can be pressed down. Additionally, moving the bone structure without reshaping it may narrow the airway, impacting breathing. These limitations have led to the adaptation of the technique into letdown rhinoplasty, where excess bone or cartilage is removed from the sidewalls, providing more flexibility and allowing the surgeon greater freedom to shape the nose without compromising nasal function.
